This form is intended for government agency requests for court records. There is no fee for records requested through this form by a government agency for official purposes.
Please provide the requesting agency's information, the applicable case number, and a complete list of the documents being requested. When possible, please include the document name and filing date for each requested document to help our office accurately locate and process your request.
Please submit a separate request for each case number. Requested records can be provided electronically to the email address listed on the request. If certified copies are required, please indicate this in the request. Certified copies may be picked up from the Clerk's Office or mailed upon request.
