This application must be filled out completely and approved by the Township before any earth change or removal of vegetation may occur within a Shoreline Protection Strip, pursuant to Section 1506,E,1 of the Township Zoning Ordinance. Paying the application fee does not grant approval.

Description of proposed alteration
A sketch plan must accompany the application detailing the proposed changes pursuant to Section 1802 in the Zoning Ordinance. This sketch shall include roads, driveways, buildings, significant natural features, yard setbacks, wells, and septic systems.

  
The removal of trees with a nine (9) inch circumference or grater (measured at a height of four (4) feet) may only be conducted under the following conditions.

  
a) The submission of a complete site plan detailing the proposed activity (including decks, patios, steps, and paths) and meeting the requirements of Article 18 of the Zoning Ordinance. Such site plan shall also illustrate existing and proposed vegetation by location and species.
b) the demonstration of improvement to a situation that is dangerous to the general public or harmful to water quality because of unchecked or potential shoreline erosion, sediment runoff, or water pollution.

I hereby authorize the submission of this application and agree to abide by any decision made in response to it. By signing I hereby give permission for the Planning Commission, Zoning Board of Appeals, Township Board, or staff to enter my property for the purpose of premise inspection and compliance of the permit.
